Non- medical personal care services
Home care services encompass Medicare Community HealthChoices waivers, Long Term Care insurances, as well as Private Duty. We facilitate care for 2-hour shifts to 24-hour around-the-clock care to provide you with the assistance you and your loved ones need. Live-in options are also available.
Case Managers
Our case managers are proficient in diverse specialized care management skills, including plans of care, scheduling, and finding the ideal caregiver for each client. They also work with the client’s medical team and family to assist in providing client-centered care.
Direct Care Worker
Our Direct Care Workers (DCWs) undergo and extensive screening process, including a competency evaluation with annual trainings, criminal history check, and have at least two satisfactory employment references. DCWs assist our clients in their homes, including facilities, with services such as:
- activities of daily living (ADLs)
- companion care
- light housekeeping
- meal preparation and feeding
- medication reminders
- assistance with medical appointments
- laundry
- transfers
- errands
Private Duty
Home Care Services - $29 per hour
Live-in – call office for current rate
